ABOUT YOU

We are looking for a Clinical Manager with at least two years post qualifying experience as a Counsellor or Psychotherapist to join us at a time of exciting growth.

You will oversee the smooth coordination of high-quality clinical work of the HCC clinic, with a focus on managing referrals and the waiting lists for assessments and ongoing therapy.

This is a part-time post (25 hours per week). It is initially for a fixed term period of one year with potential to become permanent.

Please note the advertised salary is subject to a benchmarking exercise - but the final salary will not be lower.

Embrace UK Community Support Centre is a charity based in London Borough of Haringey. We are looking to recruit an experienced Care Coordinator for our Haringey based office in London In addition to its other charitable services Embrace UK also provides domiciliary care service that provides home care support for the service users to continue living safely and independently in their own homes. We currently have an exciting opportunity for a full-time position for Care Coordinator.

Care coordinator is key to ensuring that the continuity of care for all of our service users.

Applicants must have held a Care Coordinator position in a domiciliary care agency for at least two years and have extensive knowledge of Peoples Planner and Mobizio (Care Planning) which is used to rostering and monitoring the call electronically.

* Arranging care reviews and risk assessments for service users in conjunction with the Field Care Supervisor;

* Conducting telephone monitoring and service user visits and ensuring service quality;

* Liaising with clients, local authorities and other stakeholders;

* Recruiting care workers in conjunction with HR team;

* Managing care workers and conducting care worker supervisions;

* Representing the company during meetings with officials and service commissioners if needed;

* Attending training events and ensuring that skills and knowledge of industry best practice is kept up-to-date;

* Ensuring that our service complies with CQC requirements;

* Ensuring that the highest standards of care are maintained at all times.

* Supporting Care Manager and assisting with day to day running of the branch.

Required Education, Skills, Experience, Qualifications and Essential Attributes of the Care Coordinator:

Minimum 2 years experience in Domiciliary Care Coordination

Experience in Peoples Planner. Care Planning (Mobizio) software

The role of Night Care and Support Worker to ensure the we provide our tenants with safe, compassionate and person-centred support and care. Supporting the whole care and support team, being a team player, building relationships with tenants and their families and encouraging inclusion in community activities and other social and recreational opportunities within the scheme and in the wider local environment. Care and support provide support to tenants by working alongside Care and Support colleagues to ensure the smooth running of the Retirement Living service, joining up services within the building and providing some direct care and support to tenants alongside colleagues

This role will work with colleagues on both the care and housing elements of the service , ensure the communal areas of the building are organised , well presented and in working order, working with colleagues to deliver activities and exciting social and recreational opportunities both within and outside the building and working with Team Leaders and the Independent Living Manager to ensure the service runs smoothly , delivering a great experience to tenants their families and support networks .

We are looking for candidates with positive energy who enjoy working with people and understand the importance of team working. You will need to make decisions independently and develop trusting and respectful; relationships with all members of the team and tenants and their families.

The scheme houses Older Jewish People, so you will be interested in other people, their culture and preferences for living. All training in terms of understanding the Jewish way of life is provided during our extensive induction training.

Principal accountabilities / responsibilities

You will mainly provide background support and will support care and support assistants to :

• Check tenants well-being on a regular basis, as directed according to their needs as detailed in their Care, support and Risk Plans.

• Respond to emergencies.

This includes:

• Contacting emergency services.

• Dealing with breakdown of services, e.g. power cuts, responding appropriately to death, keeping a record of all emergencies attended to.

• Providing background support

• implementing and monitoring care &amp; support plans to meet the individual needs of a diverse group of Tenants

• ensuring updates are reported for support plans to be reviewed as required.

• Support colleagues to deliver the package of care &amp; support: providing personal care, emotional and practical support to tenants and recognising when there are changes in a person's needs and report to a senior member of staff.

• Contributing to building community spirit within the resource through participating, encouraging, enabling and facilitating social events that meet the needs of diverse tenants.

• Assisting with the preparation and provision of hot meals, communally or individually in tenant flats, observing Kashrut procedures always.

• Ensure you work safely within health &amp; safety standards, raising any risks or issues in a timely manner with a Manager.

• Assisting tenants to report maintenance and repairs needs to the housing team.

• Keeping a record of all financial transactions carried out on behalf of the tenant. Contribute to effective team working and positive service culture, and be able to work effectively with people living with dementia and promote health and well-being for all tenants.

Read on to find out more about this job opportunity.We are seeking a highly motivated and flexible Band 3 Podiatry Assistant to join the Hackney Community Podiatry Service. The Podiatry Service offers a comprehensive range of Podiatry treatments to the local population.

ou will be responsible for the preparation of the clinical room, ensuring that there is adequate stock to carry out the session. They will ensure that the clinic sessions run effectively and efficiently maximizing the use of clinical time. The Podiatry Assistant will be working alongside Podiatrists.You will assist the Podiatry Team with management of the Diabetic Foot including performing woundcare under the direction of the Podiatrist. The post also includes assisting in nail surgery sessions in the community. This post also includes working at the Diabetes Foot Clinic at Hackney Hospital.

You must have an ability to work following policies, procedures and practices requiring an understanding of podiatry, acquired through on-the job training.

The Podiatry Department offers its own induction package covering all aspects of clinical care. The whole Podiatry department comes together regularly for training days and staff meetings.

We welcome applicants from individuals wishing to further their knowledge and skills and work towards “College of Podiatry assistant practitioner” qualification, which we will support.Please note the working hours for this position is hours per week

You’ll work under the supervision of a qualified podiatrist. Your clients may include older people, those recovering from injury or surgery, and people with circulation problems or diabetes.

Your day-to-day duties may include:

• preparing patients for treatment by a podiatrist

• supporting a podiatrist during a procedure

• treating patients who have already been assessed by a podiatrist

• cutting toe nails and applying dressings

• carrying out general clerical tasks and making appointments

• providing advice on foot and nail care, particularly for patients with diabetes

If you're self-employed you'll provide foot care services to the general public in private clinics, footwear chains or pharmacies.

You’ll need:

• spoken and written communication skills

• IT skills

• manual skills and good hand-eye coordination

• customer service skills

• the ability to manage your own time and work

£26000 per annum

You'll usually work 37.5 hours a week as a full-time podiatry assistant.

You'll usually wear a uniform.

Your day-to-day duties might include:

• talking to patients and preparing them for therapy

• showing patients how to use mobility aids

• demonstrating and working through exercises with patients

• setting up equipment

• keeping records of patients' progress

• providing reports to physiotherapists

You’ll need:

• excellent spoken and written communication skills

• patience, sensitivity, tolerance and tact

• the ability to encourage and motivate patients to complete their treatment programme

• an understanding and caring attitude

You’ll usually work between 37.5 and 40 hours a week. This may involve a rota or shift pattern including weekends, evenings and nights.
