Embrace UK Community Support Centre is a charity based in London Borough of Haringey. We are looking to recruit an experienced Care Coordinator for our Haringey based office in London In addition to its other charitable services Embrace UK also provides domiciliary care service that provides home care support for the service users to continue living safely and independently in their own homes. We currently have an exciting opportunity for a full-time position for Care Coordinator.

Care coordinator is key to ensuring that the continuity of care for all of our service users.

Applicants must have held a Care Coordinator position in a domiciliary care agency for at least two years and have extensive knowledge of Peoples Planner and Mobizio (Care Planning) which is used to rostering and monitoring the call electronically.

The Care Coordinator's duties will include:

* Accepting care packages and assigning care workers to service users in an appropriate manner;

* Producing care workers' rotas and managing the roster software, People's Planner

* Ensuring that service user and care worker files are complete;

* Using electronic call monitoring software , Mobizio (Care Planning) to monitor care workers' attendance and timeliness for visits;

* Arranging care reviews and risk assessments for service users in conjunction with the Field Care Supervisor;

* Conducting telephone monitoring and service user visits and ensuring service quality;

* Liaising with clients, local authorities and other stakeholders;

* Recruiting care workers in conjunction with HR team;

* Managing care workers and conducting care worker supervisions;

* Representing the company during meetings with officials and service commissioners if needed;

* Attending training events and ensuring that skills and knowledge of industry best practice is kept up-to-date;

* Ensuring that our service complies with CQC requirements;

* Ensuring that the highest standards of care are maintained at all times.

* Supporting Care Manager and assisting with day to day running of the branch.

Required Education, Skills, Experience, Qualifications and Essential Attributes of the Care Coordinator:

Minimum 2 years experience in Domiciliary Care Coordination

Experience in Peoples Planner. Care Planning (Mobizio) software

Excellent communication skills; listening, verbal and written

Great team player and team leader

Strong inter-personal, customer service and Administration skills

Flexible and adaptable

Effective organisational and multi-tasking skills

Awareness of key business principles

Excellent IT skills

Ability to work under pressure

Willing to work weekend and participate in the on-call on a rota basis

Self-motivated and proactive.

Extremely responsible and ability to take initiative.

Strong problem-solving skills and ability to think outside the box.

Well organised and Impeccable attention to detail.

High integrity and Sound judgement.

Obtained minimum Level 2 or 3 Diploma for Health and Social Care (QCF)

Full UK Driving Licence and vehicle (preferable)
